We visited jaffa restaurant on our Karlovy Vary trip. You have a lot of vegan Starters (no vegan main dishes). But with 3 starter dishes you will get a great choice of vegan food.

Nice surroundings with a good view. I thought they have a bigger vegan selection, however we ended up ordering one of the appetizers and it was filling enough.

Great middle eastern food - Edit
Asked for the mixed plate (vegetarian)to be veganized - was no problem at all and very delicious. Special mention to the cover which was hummus with pita and olives and pickles.
